One additional detail:

In version 4 of Bento, you do have to first go to the File menu and choose "Address Book and iCal Setup ..." and then check the checkbox as shown below:

If you have Bento on your Mac, it's easy.

Since Bento can create a data base of your Contacts, just choose Export from the File menu.

Choosing the Text tab allows for the creation of a .csv file. (see screenshot)
